The preoperative planning enables your eye surgeon to assess how much astigmatism is present in the cornea, and it’s orientation i.e. which direction it takes in the cornea.

Toric lens implants are lenses that are implanted internally inside the eye to replace the natural lens when it has been removed as part of a cataract operation or as part of a refractive lens exchange operation.

Toric lenses have a special shape, which means they are suited to patients who suffer from astigmatism issues that arise from a different curvature of the cornea or lens in your eye.

Unlike a standard contact lens with a spherical surface, a toric lens is a geometric shape that resembles the surface of a doughnut i.e. has a dip in the middle.

This special shape helps to create different refractive, or focusing, powers on the vertical and horizontal orientations, making it suitable to correct astigmatism.

Advantages:

  • Toric intraocular lenses (Toric IOL) can precisely correct blurry astigmatism.
  • They correct the corneal shape, which causes refractive errors.
  • Toric IOLs improve your vision significantly, usually offering patients clearer vision than they have ever experienced before.
  • They offer no additional medical risk.
  • You get to correct more than one eye condition with Toric IOLs.
  • They are designed based on specific meridians measured from the pupil like a radius is to a circle. The meridians determine the power of the lenses, which should be specific to your astigmatism.
  • Different meridians in the toric lenses also mean that the lenses cannot rotate around the eye.
  • Toric lenses offer protection from blue light known to damage the eyes progressively.

Disadvantages:

  • Given the limited range of toric lens powers, imperfect keratometry measurements, and variability of the posterior cornea, even with the best surgical technique it is impossible to completely eliminate every patient’s astigmatism.
  • Occasionally, a patient will require a second procedure to optimise the correction of astigmatism; this advance discussion can be extremely helpful when addressing an error during the postoperative period.

When a patient has a large degree of astigmatism, a toric lens would be beneficial, however, lens implant surgery (cataract surgery or refractive lens exchange), will change the shape of the cornea, and will induce or create astigmatism in the cornea, so the preoperative planning has to account for the change in the eye that the surgery will produce.

It’s very important, particularly in patients who have a very low degree of astigmatism before the surgery, that the change induced by the operation is accounted for. If it isn’t, astigmatism could be made worse.

When a patient has a significant degree of astigmatism, a toric lens IOL is an ideal option. Toric lenses come in both monofocal and multifocal varieties.

Step one: Preoperative consultation.
It’s very important in the preoperative planning to assess how much astigmatism is present in the cornea and its orientation (which direction it takes in the cornea).
Step two: Lens selection.
Your eye specialist will discuss with you the benefits of toric lenses, both monofocal and multifocal varieties.
Step three: Axis marking.
Once the patient is positioned for surgery, the eye surgeon can use a corneal meridian marker to mark the desired target axis.
Step four: Lens implantation and orientation.
Step five: Postoperative consultation.
Occasionally, a patient will require a second procedure to optimise the correction of astigmatism; this advance discussion can be discussed during the postoperative consultation.

Regular toric lenses are known to increase focus and clarity significantly for either distance or close up vision without dependence on contact lenses or glasses.

There are also multifocal toric lenses, which correct astigmatism and improve vision both for short and longsightedness.
